なつみさん２５歳の心音動画パート２です。なつみさんは普段は居酒屋さんで働いておられる方で、とても可憐な方で一言でいえば「天真爛漫」これに尽きると思います。
ずっとニコニコされている方でお目目もキラキラされており、居酒屋さんで働いているだけあって、とてもノリが良く話していて楽しい方でした。
私とは真逆です。笑
こんな子が居酒屋さんにいたらきっと毎日通っちゃいますね。
また心臓の音も細身の方であったので非常に聞こえやすかったです。
パート２では以下の内容に挑戦していただきました。
・仰向けになって圧迫したときの心臓の音
・平常時の血圧と心臓の音
・スクワット後の血圧と心臓の音
・胸のアップと心臓の音
・袋呼吸したときの心臓の音
・息を止めたときの心臓の音１
・息を止めたときの心臓の音２
・心音計で聞いた時の心臓の音
・ベッドでの診察時の心臓の音
・放〇中の心臓の音
となっております。動画の長さは３１分１４秒です。
「仰向けになって圧迫したときの心臓の音」では、圧迫中、鼓動が少し早くなっており、開放されると心臓が正常の速さに戻っていくのがしっかりと分かります。
また圧迫中、胸骨が痛いと仰られていましたので、途中で押す場所を変えております。それだけ圧迫で負荷がかかっているということなのでしょうね。

続いてなつみさんに平常時の血圧を測っていただきました。
普段あまり測らないとのことでしたので結果が楽しみとのことでした。
測ってる最中、血圧計に締め付けられて、痛たたたたたと言っておられます。
気になる結果ですが２０代女性の平均血圧とほぼ同じでした。
すごく健康的な女の子で良かったです。
その後に今度はスクワットをしていただいてから再び血圧を測っていただきました。
やはり運動は苦手なようで、「あー、きつ」と言いながらも約２分間頑張ってしておりました。
気になる血圧の方ですが、平常時より少しだけ上がっているのが分かります。
若い方なので血圧も健康で羨ましい限りです。

胸のアップと心臓の音では、なつみさんの胸ぁ鋙尾当たりをアップにして撮影をしその時の心臓の音と聴診器の動きを撮らさせていただきました。
近くで見るとなつみさんの肌はすごく、きめ細やかできれいでした。
心臓の鼓動もゆっくりで力強く非常にクリアに聞こえており、呼吸に合わせて聴診器が上下に動いているのが分かります。
またそれに合わせて鼓動するたびに聴診器が少し揺れているのも観察することができます。

「袋呼吸したときの心臓の音」では、頭に袋を被っていただき、空気が逃げないように私の手で、袋を押えその状態で呼吸をしていただきました。
少し見にくいですがパルスオキシメーターの酸素飽和濃度が初めは９６あります。最初あまり下がりませんでしたので、もう少しきつく袋を締めますと徐々に下がっていっており、それに比例してなつみさんの呼吸が荒くなり心拍数も上がっていっているのが分かります。
少しずつじわじわと苦しくなって追い詰められていっているのを見ると興奮してしまいますね。
最終的に酸素飽和濃度が８７でギブアップされており袋を外すと急いで新鮮な空気を呼吸しておりました。心拍数もMAX８９BPMを記録しております。

息を止めたときの心臓の音１は、息を思いっきり吸っていただいてからタイマーが鳴るまで息を止めていただきました。今回も呼吸をしないように私の手で鼻と口を押えています。１回目は１分間頑張って止めていただいており、タイマーが鳴って手を離すと「ぷはーー」と言って「はぁはぁ」と荒く呼吸をしていました。
２回目は２分間息こらえを頑張っていただき、途中で苦しくされており、手をバタバタとして苦しさを伝えております。
息を止めたときの心臓の音２では、息を吐いて肺の中を空にした状態で、息を止めていただいております。
１回目は１分頑張って止めていただいておりますが、苦しいらしく途中で、体を小刻みに揺らし私の手を叩いたりしています。
手を離すと貪るように息をしており、「苦しい」と言っておりました。
その後もう一度１分間頑張っていただいております。
先ほどと同じように途中で苦しくなり体をバタバタさせております。何とか１分間耐えていただきたいと思い心を鬼にしてタイマーが鳴るまで離さずにいると最後の方は、頭を横に振ったり私の手をたたいたりで、めちゃくちゃ苦しそうにしております。
呼吸を再開すると貪るように息をしています。
途中で、お腹が鳴っていたのですが、それもツボったらしくめっちゃ笑っておりました。

心音計で聞いた時の心臓の音では、初め心音計を当てていただくと、なかなか聞こえず私が聞こえる位置に当てなおしますと聞こえることができました。
その際、カメラに体が当たってしまい少し映像が乱れております。
音の方ですが、なつみさんの心臓が元気に動いているのがスピーカー越しに聞こえており、「なんか、カエルが跳ねてる音みたい」と言っておられます。モデルさんによって心音計の音の表現の仕方が違うので面白いですね。
息を止めていただくと、止めている間は音が小さくなり呼吸を再開すると音が先ほどよりも大きく聞こえております。
なつみさんも「きゃー」と言われており可愛いです。
スクワットもしていただきました。し始めるとすぐに心臓の鼓動も早く鳴っておりそれに連動して心音計から聞こえる音が大きく聞こえております。
途中で「爆音！！」と言っております。
スクワットが終わった後も音が大きくはぁーはぁーと息を切らしながら「心拍やばいです。」と言われております。心音計の表示が少し見にくいですが、心拍数もMax１３０を記録しています。
最後に深呼吸もしていただきました。少し鼓動も落ち着いてきており、心音計の音に合わせて「むぎゅ、むぎゅ、むぎゅ、むぎゅ」と言っておられました。

ベッドでの診察時の心臓の音では、パート１にあった診察時の心臓の音を今度はベッドに寝ていただいた状態でさせていただきました。もちろん服装も、なつみさんの私服でございます。ベッドに寝ている状態からスタートし、なつみさんご自身に服とヒートテックを上げていただき下着が見える状態で、聴診器を当てさせていただきました。
「ドック、ドック、ドック、ドック、」と約７０BPMで元気に鼓動しているなつみさんの心臓の音が聞こえます。
正真正銘２５歳の若い女性の心臓の鼓動なので、心行くまで堪能していただきたいです。
また途中２５分２８秒辺りで期外収縮ぽい不整脈を確認することができます。
「ドック、ドック、ド、ドック、ド、ドック、ドック」とドックの後にドと明らかに正常でない鼓動が混じっていましたので是非聞いてみてください。
他にも左肺、右肺も聴診させていただいております。呼吸するたびに「ズーーー、ズーーー」と呼吸されている音をしっかりと聞くことができます。

放〇中の心臓の音では、おし〇こ漏れそうと撮影の途中で言ってましたので、急遽取らさせていただきました。
おし〇こしていただく前に「おし〇こしたいですか？」という質問に「めっちゃしたいです。」と答えられ、「どれくらい我慢してますか？」という質問にも答えていただいております。その後水着を脱いでいただき、便座に座っていただいております。「出るよー」となつみさんが言われてから数秒後に放〇をしております。
途中、聴診器がずれており、心音が聞こえないところが少しありますが放〇している最中の心音はばっちり聞こえます。また放〇音もばっちり聞こえております。
全てだし終わってから「はぁ、出ました。」と言われすっきりされております。
